yes, i've seen 3 piece hub shells fail.

Lots of low end steel hubs that are swagged together. Try keeping a
wheel trued when the flanges are free to spin around and wobble.

Under the chain stay brake mounting

I have a 1986 or 1987 specialized rockhopper that has the rear brake
mounted under the chainstays. It's a bit a pain to adjust and
disconnect for wheel removal. Gets road grime from both wheels...

How much too low?
Not all seats are equal in height above the mounting rails.
Try a taller seat. Seats with excess padding or springs (or both) are
usually taller.
