The picture you offer shows a worn out chain on a practically new
chainwheel. You'll notice that in the picture the chain is almost a
quarter inch off pitch in half the circumference of the sprocket (at
the bottom). Where did you get such a worn chain and why are you
using it?

The chainrings look rather worn out as well if you look closely at
the shape of the teeth (although some of them do still seem quite
symmetrical).
