Default another racing cyclist dies.

The cyclists had been heading in the direction of Abergavenny and the van
had been travelling in the opposite direction.

Mr Carroll was taken to Nevill Hall hospital but later died from his
injuries.

The A40 trunk road is a dual carriageway between those points. The named
roundabout is the more westerly point, being the junction of the
London-Fishguard Trunk Road with the A465 Heads of the Valleys Road,
which is also a dual-carriageway at that point. The café is about seven
miles to the SE, along the A40 towards England.

How can anyone be on the wrong side of a dual-carriageway?

That still isn't clear about what happened, but it seems rather unlikely
that the van driver was on the wrong carriageway.

The report of a head-on crash suggests strongly that the bicycles
(allegedly up to six abreast) were the ones on the wrong carriageway.

Perhaps it'll become clearer.

"Dan took avoiding action, but went over his handle bars after breaking.
