Default Cycle friendly roundabout claims its first victim

"There will be segregated cycle lanes and the points at which riders
cross the road will be controlled by traffic lights to avoid any
potential conflict. We hope this will be a blueprint others can follow."

Perhaps the above shows the problem, cyclists do not know traffic lights
apply to bicycles.

That link is broken, it cut off before it mentioned the cyclist ignoring the traffic light.
Please post the rest of the link with evidence to support your claim.

There are a couple of obvious solutions to this

1: Keep the cycle light at green and make motorvehicleists press a button to make their light change for 10 seconds with sensors to make sure the light does not change when there are cyclists using the crossing.

2: Remove the lights and give Primary Vehicle Users absolute priority with instructions for motorvehiclists to dismount and push their toys through the crossing.
