Default Dura Ace cassette mix and match

Is it possible to put a single cog with a spacer in between the 2-cog
and the 3-cog assemblies in a Dura-Ace 9s cassette? I have read that
there may be some questions about how the shift ramps line up, but will
the spacing be correct?

Also, does anyone know about mixing SRAM cassette cogs with Shimano? I
don't even know if SRAM cassettes can be disassembled to use individual
cogs. Can a SRAM 13 or 14 be used as the smallest cog instead of the 12
that is lowest smallest cog available in their cassettes? I know that
doesn't work in Shimano but I've never seen anything on SRAM one way or
the other.

Yes.

Yes.

Unlikely. The top position sprocket has to have a built in spacer, and
it is desireable for it to also have serrations for the lock ring to
grip, though the serrations are probably not necessary if the lockring
is good and tight.
