Default STI shifter indexing, sometimes

I have an old RSX triple drivetrain and the rear derailleur shifter
will, usually when shifting up, not shift. If I shift the lever a few
times the derailleur will engage, but I am concerned that this is a
warning that the shifter is about to give up the ghost. Is this a
common problem? Any suggestions about repair/replace? Should I begin
my campaign for a new bike, using this shifter scenario as a useful
excuse?

Yes, but keep the old bike for parts...then replace the shifter later.

Yes. A flush with the 'STI repair kit', WD-40 or a good lube. If no soap,
replace is the only option.

Recommend Campagnolo ERGO, rear der and no more failed, gotta replace levers.

Are you sure the cable is not frayed where it enters the shifter?
