Proximity to immediate help; proximity to medical care. You can get many chances to die in a place like that.

You don't have to spend a lot of time examining New Yorkers to see that huffing exhaust is bad for you. If you're from a less car-infested place, you can't fail to notice.


Obviously New York City in 1900 didn't have just as good proximity to care as they do now. (Eyes rolling).

In case you missed it, New York City is the center of the news media universe. They don't appear very sickly in physical health.
